Age restrictions & ID requirements
Anyone under the age of 18 must be accompanied by a parent or legal guardian to receive any piercing related services; including jewelry inserts and removals and must provide identification, proof of age and relationship to the parent/ guardian in order to be pierced or have jewelry inserted into a piercing.
A birth certificate is required for all minors 15 and younger. A valid, government issued form of photo identification is required for both the parent and guardian. A guardian must have proof of legal guardianship.

16-17 Years old

lobes, nostril, septum, navel, eyebrow, lip, industrial, tongue, all cartilage piercings on ear, jewelry inserts/ removals
Minor: If minor has matching LAST name and ADDRESS as parent: state issued photo ID (ie. Drivers License/ Permit, ID cards)
If minor does not have matching last name and address as parent: birth certificate + state issued photo ID (ie passport, drivers license/permit, school/work ID)
Parent/ legal guardian: valid drivers license, state ID (with photo), valid passport, military ID (with photos)
Guardian: proof of legal guardianship
